Outpatients section:
This section gives reports to patients attending outpatient clinics according to the type of reports the patient wants in the following cases:
- Diagnostic catheter the Attending Physician in the outpatient clinic requests the following from the patient-an electrocardiogram-a copy of the ID card-a medical report receipt (obtained after payment of the prescribed value )
- Therapeutic catheterization, stent installation and open heart the Attending Physician in the outpatient clinic requests the following from the patient-an electrocardiogram-a copy of the ID card-receipt of a medical report-a report from the personal catheter
- The doctor asks the patient to do the following - echocardiography-a copy of the ID card-receipt of a medical report
* In all previous cases, the report is made on the form extracted according to the Ibn Sina system of hospital management, after bringing an internal medical report from the clinic on the form extracted according to the Ibn Sina System, indicating the diagnosis and treatment.
Inpatients section:

- The internal report and the rest of the papers are attached to it for signature by the deputy director of the hospital, and then it is sealed with the seal of the emblem of the Republic from the secretary of the hospital
These reports are given at the request of the patient and on his responsibility to submit them to the employers, for example, if the patient works in a government or official body
